Application to issue or replace a residence card
A lost, damaged or expiring card, or changed details. The creator walks you through the paper application field by field and gives you a ready-to-print PDF — free of charge. This is not the residence-permit application you file in MOS.
Residence-card application creator
Fill in the application to issue or replace a residence card step by step and download a ready-to-print PDF to file. It covers the card itself — this is not the temporary-residence-permit application, which you file online through the MOS portal.
- Walks you through every field in order
- Ready-to-print PDF to file
- Card issue and replacement — not temporary residence
- Free once you register a Dopomo account
An application is your most sensitive data
So security and privacy are not an add-on here — we put them first.
Uploaded files are deleted once you file
We would rather not hold sensitive data, passport scans or photos at all. We need them only while your application is being prepared. We keep them encrypted in the EU and delete them once you confirm your application was submitted — no later than 60 days after the last change to your case, with a reminder 10 days before. At that point we also delete the most sensitive data from the application. You can delete your account at any moment.
We encrypt the connection and your data
The connection to Dopomo is encrypted, and the data and files on our servers are kept in encrypted form. The keys to them are on our side, and we guard them closely.
Some data we never store at all
Answers about criminal records, and the nationality and distinguishing marks from the application form, never reach our servers — they are encrypted only in your browser and vanish when you close the tab, log out or finish the application.
You file the application, not us
We have no access to your MOS account or your Profil Zaufany. We do not sell your data — we entrust it only to the providers named in the Privacy Policy. You sign and send it yourself, when you decide everything is ready.
The CUKR card application and the temporary residence application — FAQ
What is the CUKR residence card?
The CUKR residence card is a temporary residence permit annotated "Poprzednio posiadacz ochrony czasowej" (previously a holder of temporary protection), issued for 3 years. It is for Ukrainian citizens with UKR status (PESEL UKR) who are moving from temporary protection to temporary residence.
Who can apply for the CUKR residence card?
The CUKR residence card is open to Ukrainian citizens and certain members of their families who benefit from temporary protection in Poland and hold a valid PESEL with UKR status. You also have to meet the conditions on the continuity of your UKR status.
How much does the CUKR residence card cost?
440 zł in total: 340 zł stamp duty for granting the temporary residence permit and 100 zł for issuing the residence card. Access to MOS, the system you file through, is free.
What is the difference between permanent and temporary residence?
Temporary residence is a permit for a fixed period (the CUKR card is issued for 3 years) that has to be renewed. Permanent residence is open-ended — you can apply for it only once you meet the conditions, for example after the required period of legal residence and proof of your Polish language skills.
What is the single permit for temporary residence and work?
One permit that covers both residence and work for a named employer. Part of the application is filled in by the employer — annex no. 1, with the terms of employment. Until the company completes and signs it, your application stays blocked.
How much does a temporary residence and work permit cost?
The official fees are 540 zł: 440 zł stamp duty for granting the permit and 100 zł for issuing the residence card. They go to two different accounts — you get the amounts, account numbers and payment references ready to use.
Do I need a photographer for the biometric photo?
No. You take it with your phone; we crop it, fix the background and check every official requirement. The biometric photo is included in the price of your case, whether it is the CUKR card or temporary residence.
Is the temporary residence application filed online?
Yes. The application for a temporary residence permit — including the CUKR card — is filed through the MOS portal, and access to it is free. But MOS will accept a flawed application and will not warn you about most of the mistakes: you learn about the gaps from a letter, months later. Dopomo walks you through MOS field by field.
How long does a temporary residence decision take?
The provincial office has a statutory deadline, but in practice cases run from a few months to over a year and vary between provinces. If you file on time and with no formal gaps, your stay is legal until the decision is issued — which is why the application has to be complete from the start.
Can I work while waiting for the decision?
Under the single permit you work on the terms stated in the application, for the employer who filled in annex no. 1. With the CUKR card your UKR status is the basis for working. If your situation is different, ask the assistant — the answer depends on the basis of your stay.
Can I leave Poland while waiting for the decision?
Leaving by itself does not end the case, but getting back in depends on which document lets you cross the border. The passport stamp confirming that you filed does not replace a visa or a residence card. Check your specific situation with the assistant before you travel.
What if I change employer during the procedure?
The single permit is tied to a specific employer and to the terms in annex no. 1, so changing jobs during the procedure usually means notifying the office or filing a new application with a new annex from the new company. It is one of the most common reasons decisions go wrong — do not leave it unreported.
How do I apply for a CUKR card for a child?
A parent or legal guardian files on the child's behalf. The child needs their own PESEL with UKR status and their own biometric photo — the same requirements, with some tolerance for the youngest. We run a child's case exactly like yours; each case is paid for separately.
Which documents have to be attached to the application?
Always: a current biometric photo and a scan of every page of your travel document. The rest depends on the procedure — temporary residence and work adds the employer's documents, the CUKR card adds proof of UKR status. We show a list of what is genuinely required in your situation, with nothing collected just in case.
What should I do if the office asks for missing documents?
The request has a deadline — miss it and the office can discontinue the case, and the fees you paid are gone. Paste the request into the assistant: it explains what exactly the office wants, by when you have to reply and what to attach.
What does Dopomo cost, and what is free?
The qualification check, the account, the assistant and the paper-application creator are free. You pay for a case once: 199 zł for the CUKR card, 299 zł for temporary residence and work. Official fees are paid separately, into the offices' accounts.
Does Dopomo file the application for me?
No. We have no access to your MOS account — you sign and send it yourself. We walk you through it field by field, check that nothing is missing while it can still be fixed, and stay open beside the portal as you go through MOS.
