Below are the confirmed rules for booking an appointment and collecting the residence card at the voivodeship office serving Masovia Voivodeship — separately for filing the application and for collecting the finished card. The e-services system (e-uslugi.mazowieckie.pl) requires its own account created directly in the office, and self-booking covers only selected visit categories. Marszałkowska 3/5 on a date set by the office, in person, with fingerprint verification.
CUKR card pickup — you have 60 days
You collect the CUKR card at the same office and on the same terms as any other residence card. You file the CUKR application only online in the MOS portal — there is no appointment to file — so the office visit here is only for collecting the finished card. The office will notify you of the pickup date. After the notification you have 60 days to collect it; if you don’t collect the card within that window, you won’t obtain the temporary residence permit and the card will be invalidated. You pay the fees (100 zł for issuing the card + 340 zł stamp duty = 440 zł) together with the application — nothing is due at pickup.
